Patient Information

New Patients: All patients must be referred by a physician. They can contact our office at 919-751-8444 or use our referral form. A member of our staff will call you to schedule an appointment.

It is our goal to provide you with a one-on-one informative consultation with our pain specialist. All consultations are scheduled with our NP or PA. During your consultation, you will be able to discuss your history and ask questions you may have about the treatment options available.

Please arrive 15 minute prior to your appointment time.

Bring the following to your appointment:

  • Completed new patient form
  • Insurance Card
  • Photo ID
  • All medication you are currently taking
  • Copay – We accept, cash, check, MasterCard, and Visa

Insurance we accept:

  • Blue Cross / Blue Shield
  • Cigna
  • Medicare
  • Medcost
  • Tricare Standard / Prime
  • Worker’s Compensation

Your Insurance copay is due at the time of check-in.

For prescription refills, call our office and leave a message on our refill line. Request are to be called in 48 hours prior to pick up. Please leave your name, date of birth, name of medication, and date that you are requesting pick up. We will not call you to let you know your prescription is ready to be pick up. If you request a refill be done in less than 48 hours, we will call you if we are NOT able to do this.
